The condition you are suffering from is known as phimosis. You can have sex even with phimosis and mot of the patents do not experience any problems. however, some may have tear of the foreskin. Tear of foreskin can happen even in those with no phimosis.
However, you should still get the phimosis operated upon. As body excretes it waste material as sweat or urine that same way penis excretes its waste material as smegma. Smegma gets collected under the foreskin and should be cleaned everyday. Prolonged exposure to smegma leads to increased chances of developing cancer of the penis later in life,. This very fact should be good enough reason for you to get operated.
There are two surgical options available to you- dorsal slit and circumcision. In dorsal slit a cut is made at the top and the foreskin becomes like open book and hence the area can be cleaned everyday.
The pros of the two procedures far outweigh the cons and hence they need to be discussed. Either of the two procedures does not affect sexual life. In fact studies have shown that circumcision leads to delayed ejaculation and hence more enjoyable sex.

Both are equally good.
Dorsal slit allows you to go back to work the same day but it leaves an unusual shape on penis and there is 10 percent chance of recurrence.
Circumcision is one time solution but you are off work for 5-7 days.

Dear Sir,
The circumcision can be done under local anesthesia as well as general anesthesia. It is not painful in either of the two. However, you may experience pain post surgery when effect if anesthesia goes away.
