Enlarged PITUITARY, CHRONIC INFACRT in basal ganglia in CT.

Resolved Question:

CT BRAIN (PLAIN & CONTRAST)

Finding:
Motion artifacts noted grossly limiting evaluation.
1.chronic lacunar infarct noted in left basal ganglia.
2.small ill-defined hypodensities are noted in the periventricular and deep white matte region, likely respresenting small vessel ischemic disease.
3.The ventricles, cisterns, sulci and cerebellar foliae are mildly prominent consistent with generalised cerebral and cerebellar volume loss.
4.No obvious intracranial hemorrhage or midline shift.
5.Rest of the cerebral hemispheres and basal ganglia demonstrate normal attenuation.
6.Pituitary gland appears enlarged on size with heterogeneous post contrast enhancement.however evaluation limited due to lack of reformats.
7.The cerebellar hemispheres and brain stem are otherwise unremarkable.
8.The cerebello-pontine angles appear normal.
9.The ventricles, sulci and basal cisterns are otherwise unremarkable.
10.The calvarium is unremarkable. No obvious fracture.
11.The mastoid air cells and visualised paranasal sinuses are clear.
IMPRESSION :
1.No obvious CT features of meningitis.
2.Enlarged heterogeneously enhancing pituitary gland as described, of concern for pituitary adenoma. Evaluation limited due to lack of reformats. Recommend MRI for further evaluation.
3.chronic lacunar infarct in left basal ganglia.
4.small vessel ischemic disease.
5.Age related neuroparechymal volume loss.

His cpk (creatine phosphokinase) level is 1037,
He is taking clopidogrel 75mg 0-1-0

aspirin 150 mg 0-1-0,
Atorvastatin -20 mg 0-0-1,
Glimepride - 1mg 1-0-1,
Metformin-500mg 1-0-1,
Metoprolol tartrate -50 mg 1-0-1,
He is diabetic, have cholesterol, and pressure .all are normal

I do not have the symptoms of your dad. This is because the investigation reports need to be correlated with patient's symptoms.
If you are looking for opinion on the reports, here it goes.
CT scan of brain shows areas of decreased blood flow, for which he is already of blood thinners such as clopidogrel and aspirin.
In addiiton, there is a doubt about pituitary gland timor, which needs to be confirmed or excluded by doing an MRI scan of brain.
CPK level is high, which could be related to intake of atorvastatin tablets, if he has muscle pains.

Medical report is provided, Doc please tell his current condition and treatment, medicine's to be consumed and diet


In the medical report provided, I found two abnormalities.

The first is high Uric acid, for which allopurinol tablets are needed.

Second is cholelithiasis (stones in gall bladder). This needs to be shown to surgical gastroenterologist to decide about cholecystectomy (surgery to remove gall bladder).
