Anxiety, depression with AGAROPHOBIA and bed wetting. Treatment?

Resolved Question:

Hi I am a 44 year old male non smoker I suffer with anxiety and depression and agoraphobia (not the reason for my question but background information) june of 2013 I was in the shower blacked out and woke up on the floor directly prior to the black out I had a odd deja vu feeling then woke up on the floor after this I went to see a GP and he did blood tests and he found I had thyroid disease ( hypo) I am on Levothyroxin 75mg once a day for that. we didnt go into the blackout much further about 6 months later it happened again right after I woke up I was walking to the bathroom and got the deja vu feeling so I sat down in a chair but woke up on the floor. I went back to him and he said he he didnt really have an idea what coudl be goin on at the time and I kind of pushed it aside then few months later I got the deja vu feeling again I sat down and did not actually pass out I had a odd sensation in my head and body then got nauseous afterwards. 2 months after that it happened again same type thing never another black out. I then started having about one of these episodes a month usually while doing mundane things ( I work out daily I do 10 miles a day on exercise bike and I lift weights twice a week never any episodes during strenuous activity) back in oct of last year I had 2 of them 2 days in a row none of these since the first 2 have I blacked out usually the deja vu( aura) feeling then the electrical sensation and nausea. Due to my anxiety issues I am dont like going to doctors and usually put things off. but I finally went back to my GP in dec and we discussed it and he said he thinks I am having petit mal seizures. so he prescribed me Ethosuximide 250 mg I beg taking it the 2nd day on it I had very bad headaches and dizzy so he told me to just take it every other day and build up to it so I did that for 2 weeks then I went to taking 1 250mg dose everyday. ( I also take a lot of vitamins and supplements due to trying to be healthier) last monday I began taking 250mg twice a day the day after I started the 250 twice a day when I tried to go to sleep( I have insomnia and have for 20+ years I usually go to bed around 4-5am and sleep till 11am-12pm if I am lucky) I kept having the aura feeling so I stayed awake for 2 days then I called his office and he said to go back to the 1 pill a day( I skipped a dose as I was hoping it would help get it out of my system faster) I still could not sleep as when I tried I would have the aura and seizure feelings while trying to fall asleep. Wends I went into his office as I was highly agitated and upset from no sleep. He gave me some xanax to try and calm me down( I was on xanax as well as other meds for many years for my anxiety but have been off them for 7 years now due to moving and not finding a new doctor to prescribe) I came home took 1mg and slept for about 8 hrs. I also kept having really bad night sweats I would wake up soaking wet to the point my bed sheets were wet.when I tried to sleep again on thursday night I could not fall asleep with the 1mg of xanax I kept waking up due to the aura feelings. so friday morning went back into his office and discussed it with him and he said that we should try me on Depakote but I told him I had heard bad things about the side effects so I told him that I dont think I wanted to try it so he said at this point he thinks I need to find a neurologist. At the same time I asked him about the Ethosuximide and he said stop taking it. So I did not take it on thursday or friday I have not been feeling normal I am kind of foggy feeling and my head feels like ice water is being poured on it. Last night I got to thinking that maybe I should not have just stopped taking the meds cold turkey so I called my local hospital and the pharmacist said NO I should not have just stopped cold turkey. So I took a dose at around 2:30 am est. I tried to sleep last night I took 3 5mg melatonin to try and help me sleep as I dont want to get back use to taking the xanax as he only prescribed me 20 .5mg but after I could not sleep with the melatonin I kept having the aura and some twitching which I dont remember having up until this past week ( I had never had any of the aura or seizures etc until I went on the 250mg of ethosuxmide to my knowledge just my normal issues of hard to fall asleep) I did not know if I was in some type of withdrawls from the Ethosuxmide from the missed doses or if all of the new odd feelings I am having are something new as prior to this week I felt normal except for headaches and some constipation and diarrhea with I had read is a side effect of the Ethosuxmide. I guess my question is could all of this be being caused by the Ethosuximide or could something else be going on due to the new feelings of the new seizures when I try and sleep and the odd sensation in my head also at times for the past 2 days. I do know that my anxiety issues will play a part in some of this but I have never had it do any of this. I am tryin to make up my mind if I need to go to the ER prior to trying to find a neurologist and etc. please respond asap as I am holding off on the ER till I hear back

Thanks

Hello,

Thank you for posting your query at DoctorSpring.com

I have noted your symptoms, medicines used, and past medical History.

At this point of time, I feel you are suffering from seizures, and need detailed evaluation by a neurologist.

Investigations would include MRI brain, long term video EEG monitoring, and if needed, a PET CT of brain.

The likely medicine to be effective in your case is sodium valproate and it is safe and effective.

I agree that ethosuximide and anxiety have contributed to your problems, but they are not the main issues right now.

Also, you can wait for a neurologist appointment and there is no need to visit ER right now.

Best wishes,

HI

I went to the ER being I was just not feeling right they did a full blood work up and found nothing and also did a CT scan and found nothing with that as well and told me to get intoouch with a neurologist and gave me some ambien to take to try and help me sleep but I took it last night when I got back from the ER at around 1am and still did not get much sleep as I keep having the aura feels when I close my eyes to sleep...

Thanks

Thank you for getting back.

Normal blood work and CT scan is good news.

For good sleep, you can ask your doctor to prescribe zolpidem tablets.

Best wishes,
