It looks like you are worried about her fracture clavicle of left side.
There are some points which you have mentioned here & which are of importance-
-She is not having pain or very little pain at present.
-Skin poking is present from one fragment.
-Her arm is doing well ,the only complaint is minor numbness at thumb.
Most of the clavicle fracture unite well ,if kept in figure of eight or clavicle brace in reasonable apposition position.In your case case injury is also 4 weeks old.Most of the old surgeries become difficult to treat even in young person.
Looking at all points in such case i will prefer to keep her on conservative treatment.
In her case the nailing option is not feasible as injury is old & also her age is more.In this if you will decide for surgery then you should go for plating option.
You should ask following questions from your surgeon-
-Is this surgery is affected by other injuries of chest?
-Is this surgery needs bone grafting?
-Is there need of osteotomy of clavicle bone?
-How much time it will take for recovery?
-What are the complications?
-Is numbness at her thumb is due to this fracture?
She should take a second opinion from other unrelated surgeon also.
