PCOS is not known to cause such severe pain.
The ' cysts ' in PCOS are just small collapsed ovarian follicles.
PCOS can cause painful periods, heavy periods, irregular periods , but pain per se, is not characteristic of PCOS.
WHen was the last scan she had ?
What is her height and weight ?
How long has she been on birth control ?
I think this could be a sign of a bigger solitary cyst on the left ovary.
Again, could be a mild bowel infection / constipation .
If she has had unprotected sex, and the period that she had lately was not average, then a pregnancy test should be in order to rule out an ectopic pregnancy.
IBS should always be thought of.
With two such episodes now, a thorough evaluation is necessary.
You need not panic , but yes, PCOS alone does not explain the pain.

Patient replied :
The last time she had a scan was in July and all it did was show that she had PCOS. We have scheduled a doctors appointment this coming monday to see if he can determine the exact issue, but I will also have her take a pregnancy test (Just in case) Would a home test kit work? And thank you so much for your advice and insight, this is definently a great website!

A home test kit would work fine until you get to the doctor and have a pelvic scan, and a blood pregnancy test if the scan indicates that.
