Hello,

I am a 27 year old female with lots of crypts in my tonsils. I get tonsilitis or strep about 5 times/year sometimes more, sometimes less. This past September I got tonsilitis twice. On February 2nd I went to the doctor with a really sore throat and fever and they diagnosed me with strep (without a strep test). They gave me penicillin (10 days worth to be taken twice per day). Tuesday, Wednesday and Thursday I slept in 2 minute increments because i would wake up to spit out my saliva (because it hurt to swallow /saliva production felt more than typical). I was off and on feeling a bit better some days and not others (usually worse on days I did too much; I work full time and am in school). Had a fever for a good part of that week. On day 10 of penicillin, I still was t 100%... For 4 days afterwards, I tried to rest /drink as much as I could (sleeping most days about 12 hours), but by Tuesday had a really sore throat all over again. Went back to the doctors who Once again diagnosed me with strep throat (without a strep test) and gave me azythromicyn (250 mg) at 1 pill per day for 5 days (with 2 pills on first day).. Thursday night I was starting to feel better but broke out in a rash on the backside of my thighs that was itchy. Went to the clinic that night and the doc said that it was eczema and was prescribed hydrocortisone. By Friday evening, my whole body wAs covered in this rash. I went back to the clinic where they tested me for mono, strep tthroat and gonorhea. Mono test came back negative. Waiting on others. The doc told me to stop taking azythromicyn in case I was allergic to it. I still had a rash Sunday (although only half my body was covered). I went for a walk outside in -10 Celsius weather (which I normally do to walk my dog) and after 10 minutes broke out with a red and hot slightly puffy face and upper neck, the rash on my legs got even worse. The lymph nodes on the back of my head were enlarged (not sure when that started). But off and on had swelling in different spots on my head (eyes, behind right ear, throat, behind head) over the course of the past 3 weeks also felt and am still feeling a bit lithargic. I took Benadryl last night after my walk (this past Sunday) and fell asleep. Had another 12 hour sleep. Rash today (Monday feb 23) is much better! Still itchy but only on my legs. I still have (and have always had this past 3 weeks) white patches on my tonsils.. White spots are a little worse today than yesterday but no pain or fever or any other symptoms besides rash on thighs and feeling a bit lithargic.

Wondering whats I need to do to get better? Any suggestions would be great!

You are suffering from recurrent chronic tonsillitis. Now you had two episodes and last one was follicular tonsillitis and on treatment with azithromycin you had drug rash which is improving now.

With next episode you should do one throat swab for pus culture and sensitivity. That will help in deciding correct antibiotic. Otherwise first line drug is amoxicillin. So next time you can start amoxicillin with clavulanic acid 625 mg twice daily for 5 days after discussion with your Ent specialist.

More then 5 episode of tonsillitis in a year is an indication for tonsillectomy surgery also. But it is an elective procedure , so we can wait and watch and can plan later also.
