Dear Doctor,

For about two years I have had mainly right sided testicular pain which comes and goes. It is more of a dull ache and feels as if it is coming from behind the right testicle, pretty much where the epididymitis is.
I am too embarrassed to seek face to face advice for this.

I am 45 years old, happily married for the past 13 years.
I have no discharge.
No painful urination.
I have no fevers associated with the pain.
It appears to me there is no lump or swelling. Both testicles seem to be of equal size.
There is no redness associated to the scrotum etc.
I don't wear tight fitting underwear or trousers.

I really don't want to go through the embarrassment of a physical exam and even more so, an ultrasound scan.

I am otherwise in good shape, correct weight for height etc. I do have a very sore back without any sciatica or other neuro defect. It is muscular pain and does cause me a lot of problems lately. I have had a CT Lumbar Spine done which demonstrated slight bulging of a disc L5/S1 but due to CT, cannot comment whether this has any impingement on the spinal nerve. To me it is all muscular strain and my main concern is the aforementioned problem.
