Resolved question:
Symptoms: right eye weeps and red, nose gets blocked, pain radiates from right shoulder to the back of right ear and head feels as if it is being drilled, and pain in right temple. My right eye drooped after I had suffered this headache and is still drooping and it is one week later. My eyelid feels as if it is really puffy and I sometimes don't have any control when it closes. I have had a few episodes and it is mainly when i fall asleep...the pain is so severe that i have to virtually squeeze my head to ease the pain...I am not right after the episodes, I feel lethargic, sometimes I get moody and my hairline above my right brow feels like it has been cut open that kind of sensation..

Hello.
Thank you for your query at DoctorSpring.com.
I understand your concern and will do my best to help you.
I would require additional information in order to provide my best opinion.
I presume you have had the headache a week ago. Am I correct? Do you still have the headache now?
- How long did the headache last?
- Any aggravating and relieving factors?
- How long do the associated symptoms like the red weepy eye, blocked nose, shoulder pain last ?
- Are both your pupils of the same size especially in dim light?
- Any history of trauma to the head or neck?
- Any history of similar headaches in the past?
- Any medical ilnesses?
Please reply to the above in the follow-up.
Awaiting your response,
Kind regards.
I had the headache a week ago. and I do still have head pain now, my scalp is very tender and the headache is dull. I feel a very weird sensation around my hairline above right eye, it feels as if the skin has been cut.
The initial severe headache had came twice (2 nights in a row) and they last for around 2 to 3 hours.
- Any aggravating and relieving factors? the only relief i get is by taking strong pain killers, aggravated pain usually radiates from the harsh pain in the tip of my right shoulder and right top of arm.
- How long do the associated symptoms like the red weepy eye, blocked nose, shoulder pain last ? the blocked nose is usually during my sleep, my right eye lid is still swelling and drooped. it feels like it is pussed, it sometimes has to be relieved by me lifting the eyelid up.
- Are both your pupils of the same size especially in dim light? pupil is slightly smaller than the left pupil.
- Any history of trauma to the head or neck? I haven't had any trauma to head or neck.
- Any history of similar headaches in the past? only normal headaches but in the past I have had sometimes heard a loud bang inside my head during my sleep which wakes me up.
- Any medical ilnesses?
I have hyperthyroidism, I have just been diagnosed with a problem in my right kidney, it is not functioning due to a blocked tube that goes from the bladder to the kidney, the kidney is possibly going to be removed as I have been told;
Another symptom that I seem to be getting is the hissing in the ears, its like it is a fuzzy hissing. I get high temps as well sporadically.
I have had a radical hysterectomy due to endometriosis and fibroids about 12 years old patientago.
Thank you
Marie
Hello Ms. Marie.
Thank you for the additional information.
You seem to have had a cluster headache. These are a rare type of headaches which can last untreated for 15 to 180 minutes. The headache is excruciating and usually is one-sided and may involve tearing and redness of the eye, a stuffy nose, eyelid edema, drooping of the eye lid and a smaller pupil. All of these changes (autonomic changes) occur on the painful side. They usually disappear with the headache but in some patients, like in your case, may persist indefinitely. Sweating and blood flow to the skin can also increase on the painful side.
The diagnosis of this entirely clinical but neuroimaging with a cranial CT scan or a cranial MRI study should be done to exclude abnormalities of the brain and pituitary gland.
Treatment for an acute attack is usually with subcutaneous sumatriptan and oxygen inhalation.
Verapamil is the drug of choice for the preventive therapy of cluster headache.
Also, smoking, alcohol use, certain foods, and other things that trigger your headaches should be avoided.
So I suggest that you please consult your doctor for further evaluation and treatment. Consultation with a neurologist or a headache specialist may be required.
I hope this was helpful. Feel free to ask queries.
Kind regards.
