Prolonged bleeding with history of PCOD

Hi Doctor. I have had prolonged bleeding for past 60 days. initially i thought it was due to stress(there was a lot of emotional stress situation). Then I checked with an ayurvedic doctor. Bleeding stopped for 3 days and then restarted. Flow is not heavy so am not able to ascertain if its heavy spotting or actual periods. I have polycystic overies but have not had problems with periods before 2012. Got married in Apr 2012 and had to take medicine to defer periods twice. Once in Apr'12. Second time in Nov'12 but that did NOT stop the bleeding. After that incident am afraid to go to doctor beacuse from Apr'12 onwards I have not had regular periods and feel its because I was given wrong medication or something. I have gotten really worried now. Please help me. I got medical tests done day before yesterday.

Thanks for your query at DoctorSpring. I have gone through your reports.

Bleeding that goes on for 60 days at a stretch should definitely be investigated. You need to consult a Gynecologist straight away.

I suspect that it is the PCOD that is responsible for this situation. In PCOD, due to hormonal imbalance, the uterine lining does not get shed off regularly, and it builds up, then sheds erratically leading to such bleeding pattern.

This is called endometrial hyperplasia ( thickening of the lining of the uterus ), commonly seen hand in hand with PCOD. Of course, anxiety, stress, being on birth control or other hormonal medications etc. are common reasons.
Your thyroid functions are normal, so that is ruled out.

Apart from the tests done, I suggest -
1. Pelvic ultrasound done transvaginally.
2. Thorough internal per vaginam and per speculum check up by a gynecologist.
3. PCOD panel - FSH, LH, DHEAS, free testosterone, Fasting serum insulin
4. Proper glucose tolerance test ( as your average blood glucose and HBA1C are raised )
5. Gradual and sincere weight loss programme
6. Vitamin D supplements as you are deficient.

You might need cyclical progesterone every month regularly to stabilize your cycles.
