Irregular cycles and spotting is interlinked. You should be evaluated for the causes of irregular cycles, so that you do not face any issues in childbirth subsequently in the future.
Get the following investigations done -
FSH, LH, TSH, Prolactin, DHEAS, Free testosterone, Lipid Profile, OGTT, Pelvic ultrasound scan.
That should show a diagnosis, generally I suspect PCOS ( polycystic ovarian syndrome to be the cause ).
The UTIs could be due to a lingering infection in the kidneys or bladder - please get an ultrasound of this region done and complete urine testing to rule out any stone / cyst.
Also, an internal evaluation by a gynecologist to rule out a pelvic / genital infection that could be causing recurrent UTIs.
Please get your calcium and vitamin D levels checked.
Regarding the pain, please take a pregnancy test as with irregular cycles, you should rule out a pregnancy, miscarriage or an ectopic pregnancy.
Also it could be an ovarian cyst that could have ruptured. Appendicitis is also likely.
Do not ignore such severe pain and please have an evaluation along the lines suggested.
