The ideal treatment for your husband is removal of urinary bladder along with urinary diversion. The diversion can be ileal conduit or neobladder.

Neobladder is safe in high grade bladder cancer as long as the urethral margin doesn't have cancer.

'Surgery is the best modality in case of recurrence in bladder cancer. The procedure which is usually performed in such case is called radical cystectomy wherein the bladder is surgically removed and pouch replaced by another organ to hold urine, routinely with colon.'
