I have gone through your question in detail and I can understand what you are going through.
If your doctor is treating you for depression then you have all the rights to recieve a certificate that you are seeking treatment for depression. However the doctor reserves the right to mention or not about the requirement of staying out of school. If the doctor has advised to stay out of school then he should give you a certificate stating so. Even if not , atleast he can give a certificate that you were on treatment for depression for the said period.
You may give him a written request for certificate and ask him to refuse on paper if he wants to.

Thank you for your answer and help. I am definitely going to ask my doctor for a certificate in writing. This way I can atleast get a solid answer. Is there any particular way I should write my letter? I apologize for this being more of a medical advice question instead of a health question. I just did not know who else to ask.

Thanks for a follow-up.
You may mention your consultation dates and the reason for sonsultation in brief. And then request for a certificate mentioning the purpose for certificate. You may also mention that if the certificate is being denied, it be denied on paper by replying to this mail.
