Decrease in ejaculation and pain in penis after ACUTE PROSTATITIS.

Greetings: My Name Is Brian L. Jackson

I suffered from acute prostatitis with sepsis in ___. The symptoms were
pelvic pain (near the anus), high temperature, and chills with extreme
trembling. I was concerned more about the pain and the trembling than
anything else, because I had never even heard of prostatitis or sepsis.

I was in the hospital for a few days and was administered Ciprofloxacin.
After I went home I took more Cipro for 28 days.

Since that time, during intercourse, I have had a decrease the force
of ejaculation. The amount of "pre-come" (to use the vernacular) is
excessive--like it "leaks" out--and sometimes there is none to very
little ejaculate at orgasm.

After I partially ejaculate, I feel as though I am not fully empty;
also, there is a dull ache in my lower back- which goes away in a
few seconds.

Later, I can feel the ache return--but if I massage my penis (push from
the base to the tip) I am sometimes able to remove more of the ejaculate -
and again the ache goes away.

I have heard that it is even possible that some of the semen can "back up"
into the bladder; is this possible?

I just would like your opinion of these symptoms and whether they can
be caused by the prostatitis; I also would appreciate any other comments
you may have--thanks!

Dear Mr. Jackson, Pain in the lower back as well as the penis can be present after an episode of acute prostatitis. The alteration in the amount of pre-ejaculate(precum) is also secondary to prostatitis. The decrease in ejaculate is caused due to retrograde ejaculation, where the ejaculate goes into bladder instead of coming out and it is nothing to worry about. It shall improve by itself. I am interested to know what other medications you are taking other than ciprofloxacin. Please feel free to ask queries.

Thank you for your reply! Three more things:
1) I meant to say I was in the hospital in 2011; there should not have been a blank (I wrote it and cut and pasted the question)
2) You mentioned something called "Retrograde Ejaculation:" sometimes when I urinate, I see a white film floating in the toilet water: could this be residuum of excess sperm based on this condition?
3) Is this why if I perform a massage on the urethra of my penis (in a downward motion) it releases the rest of the sperm and relinquishes the pain I feel?

Dear Mr. Jackson,
I understand you suffered from Prostatitis in 2011. The foam in your urine is not because of sperms because of retrograde ejaculation. Foam could be due to the presence of proteins. You should undergo urinalysis to find if there is any protein in urine. The milking of the urethra leads to expulsion of urethral secretions and has no relation to retrograde ejaculation. Sperms which have undergone retrograde flow can be passed with urine only and they are indistinct and cannot be differentiated in the urine.
As I have suggested, please undergo a urine analysis.
