I am a little confused. Your sex is male and you are producing semen but the ultrasound report shows uterus and ovaries. Both the above mentioned organs are found in females. You have probably got somebody else report. Can you please confirm??

Yes sir i meant vaginal dischage only Mostly when i eat something which increases stomach heat
You don't have any major issue. The adnexal cyst is a follicular cyst. Such cysts keep on forming midcycle in all menstruating females. No treatment is needed for the same and it disappears be itself.
For the yellowish discharge, You seem to be having pelvic inflammatory disease. You should avoid intercourse for few weeks. Start yourself on t. Doxycycline 100 mg twice daily for 2 weeks and combination of ofloxacin and tinidazole for 5 days under the care of your doctor.
Kindly take the medicines and you will be fine.
