Since you wore an extra condom, it automatically reduces the chances of getting HIV by 85 percent. If the condom was worn properly, then the incidence is even lesser. Regarding the tear you will be able to understand once you pull out, because semen will start spilling through the tear. However 2 condoms completely reduces the chances of that too. So it is safe to say you can't get HIV from her through sexual intercourse, since you had adequate preotection on.
However sometimes, HIV positive people can have other lesions in their mucosal layers, like minor cuts/ wounds, and they can still transmit HIV from such cuts if it comes in contact with a susceptible area in your scrotum/skin. Again the incidence for this is very low, more of a theoretical possibility.
To answer your last question, amazing progress has been made in the field of HIV. If detected in earlier stage, it can still be controlled with medicines and people can lead a normal life for a long time. With Type 1 if someone has HIV too, it will be difficult since that person will be very prone for infections. But do not be unnecessarily worried at this stage.
I will advise you to get a 4th generation HIV test done at the end of three weeks. It should come out to be negative, and once it does, it will completely rule out HIV.

I am sorry i thought you were wearing double condom initially. As i told you before, since you had a condom on you absolutely negated the chances of getting HIV from her. It is theoretically possible, to get infected through condom tears, but till now i have never come across such a case. So i will advise you not to worry.
Coming to the investigation. Previously we had only ELISA and WESTERN BLOT to test HIV, and it was found that sometimes it used to take around 3 months for the virus load to reach a level in the serum that could have been detected. However in the last decade, medical science has progressed a lot and now we recommend the HIV 4th generation antigen-antibody combo test, which is positive by maximum 3 weeks and has 95 % sensitivity. Even more in some studies. Since its a combo test if it misses the antigen, then it will definitely pick up the antibody. I will advise not to search internet and trust all its articles because some have wrong facts and they can mislead you. You can trust me on this and get yourself tested after 3-4 weeks, if its negative you have a conclusive evidence too.
It is definitely possible to get HIV from an infected female, through the contact with her body fluids or vaginal fluids, even though the reverse is more, that is a man infected with HIV has more chances to pass it on to the female partner, still even infected females DO pass on the infection to men. I won't be worried in your case since you had a condom on.
Please get yourself tested, so that more than anything your anxiety will be gone.

Regarding your friend, if the prostitute was tested three weeks back and was negative, then even if she had intercourse and got HIV from someone else, it is unlikely to be able to have enough build up of HIV RNA within 3 weeks to pass it on to someone else. But again you can never predict. If she had sex with someone who has highly infectious at that stage, then chances are high of passing on the infection. Please ask him to get himself tested after 3 weeks with the 4th generation test. I will once again reaffirm that it is the only test that can pick up HIV in 3 weeks and it is 95 prercent sensitive.
Regarding your second question, i think you probably referred some article which mislead you. This is the thing. If a man is positive with HIV, and has unprotected intercourse with someone who is negative, the semen has very high percentage of HIV viral load compared to any other body fluid, so passing on the infection is very likely. Regarding women, since they don't really have active discharge like men of that volume, the incidence of them passing on to men, is slightly lesser but still enough to make sure people use protection all the time. They can transmit the fluid through their vaginal fluid, or even through minor wounds in their mucosa, which are highly infectious spots.
