I can understand your concern.
I reviewed your history carefully. You mentioned about getting pain and swelling on the shoulder and then following rash n itching. Also about bruises on the palms. Bruising on the palms could suggest a condition called urticarial vasculitis.
Also you have a history of heartburn. All this could point to an internal condition which is causing the urticaria.
I would recommend that you need to consult a physician to correlate the symptoms and come to a diagnosis. And then accordingly treat it.
Coming to the urticaria, it can occur due to various causes. Sometimes an external cause or sometimes internal. Its important to resolve the causative factor to make sure the urticaria doesn't keep recurring.
But it's also important to control the rash and the itching so it doesn't cause discomfort.
I recommend taking anti histamines like tab. Allegra 180 mg once in the morning and tab. Claritin 10 mg at night, along with tab. Ranitidine (zantac) for 2 weeks. Regular follow up with your dermatologist to monitor the improvement of the rash and updosing or tapering of medicines as required.

Thank you for your reply. My question was also accompanied by blood report and urine report. Do you see any anamolies I these that I need to be concerned about?

I checked the reports but nothing abnormal in that. I don't recommend any other tests right now but in case your physician does it would be best to get them done.
