Hello. Thank you for the follow up. I have gone through all your investigation. Seems to have word and epilepsy attack. This could be due to an abnormality in the heart rhythm such as intermittent and transient complete heart block. This could be due to brain abnormality, brain evaluation has not revealed any abnormality. His 24 hour holter examination also has not shown any complete heart block. However the event of heart block could have been a transient event which did not occur again during the 24 hour holter examination. In a situation given the age of 83 years old patientit is quite likely that the patient have suffered from an attack of complete heart block attack. So even though the test could not prove that he had a complete heart block it is reasonable that he undergoes a permanent pacemaker implantation.
