I understand your concern.
You're suffering from heartburn and hyperacidity, a mild form of GERD. It can be completely cured, so please do not worry. A lof of factors make us prone for hyperacidity, for example change in diet, change in food timings, increased gap intervals between meals, lack of physical exercise, stress, e.t.c
I will advise the following medicines for you. Please continue them for about a week and let me know if there is improvement.
1) Cap. Omeprazole 20 mg once daily before breakfast. For 2 weeks.
2) Syrup Sucralfate 2 teaspoons after every meal for 1 week.
3) Tab. Lactobacillus twice daily 1-0-1 for 1 week.
These are all OTC medications, so you can get it from the pharmacy directly.

Yes you will get the medicines without prescription as all of them are OTC drugs.
Regarding foods, try to avoid spicy food, fatty food, vegetables such as Cabbage, cauliflower, broccoli, dairy products ( skimmed milk is a better option ), bread, e.t.c
Have more of green leafy vegetables, fruits, oats, cereals, soups, e.t.c

Yes, that is the correct Lactobacillus (acidophilus) that you are taking.
Pentab dsr and Omeprazole are essentially containing the same medicine, therefore take either one but not both. Whichever tablet you chose to take, take 1 a day, early in the morning before you have eaten anything.
The sucralfate tablets will also work in place of syrup, take 1 tablet on empty stomach 1 hour before each meal and at bedtime.

I hope the dosages are clear in this way
1) Cap. Omeprazole 20 mg once daily before breakfast. Take for 2 weeks.
2) Tab Sucralfate since this is a 1g tablet (yes the picture you have sent is the correct one) take it twice in a day, on an empty stomach for 1 week
3) Tab. Lactobacillus twice daily 1-0-1 for 1 week. (morning and evening everyday for 1 week)
