I can think of the following -
1. If you are sexually active, get an STD screen for yourself and your partner.
2. Get your blood sugars tested, diabetes predisposes to recurrent infections.
3. Do not cleanse, douche, rub or scrub yourself vigorously. Refrain from perfumed and medicated soaps, cleansers, tampons, tissue paper, gels - all these interfere with the normal vaginal pH, making it prone to infections. Wear cotton undergarments, changing often to avoid dampness. Just clean the private parts with lukewarm water and pat dry gently.
4. Have protected sex till you get your results of the STD panel.
5. Get an internal examination - per vaginum and per speculum. Also a vaginal swab to confirm the exact infection. A pelvic ultrasound scan is in order as sometimes the infection maybe deep seated in the uterus. All your symptoms are highly suggestive of a lingering resistant infection.
6. Always urinate after sex.
7. Drink atleast 4 - 5 litres of fluids daily.
8. Get a urine culture done, and take a proper round of culture based antibiotics.
9, Try cranberry juice - taken regularly, it works wonders for the UTI

Just a few more questions....
I have had a vaginal examination, i dont use anything scented, i have had a urine culture, and have been tested for all STDs and still have not found answers.
Do you have any ideas of what it could be?

You then are just more prone to repeated infections.
That sometimes happens, just as some people are more prone to flu and common cold.
It will help to drink atleast 4 - 5 litres of fluids daily, will keep your urine flushed and clean.
Also, please take a course of Probiotics for 3 months, that will improve the vaginal flora, and make it resistant to infections.
