Reasons and treatment for elevated BLOOD EOSINOPHIL LEVELS.

Doctor,

from blood test report of my son, it is observed that eosinophil count in WBC is 10%, would like to know cause of excess eosinophil in WBC and possible effect. What medicine to used to control.

Blood report:
Haemoglobin : 15.3 gm/dl
total Leucocyte count ; 4800 cell/cu.mm

Differential Count
Neutrophils : 60%
Lymphocytes : 29 %
Monocytes : 01 %
Eosinophils : 10 %
Basophils : 00 %
Platelet count : adequate

Blood Picture
R.B.C : Normocytic/Normochromic
W.B.C : Eosinophilla present

Age of child : 13 years
General Phy.: Height : 4.9 ft.
Structure : pale body with little mussel
diet : he take little diet and he feel exhausted after few work
complaints : pain in LEFT foot sole.

I have gone through your son's history and complaint. At his age, his height is appropriate and at 13 years, he is still a child and less muscle mass is nothing to worry about. However, his dietary habits, easy fatigability, pain in the sole and increased eosinophil count are a cause for concern.

Increased eosinophil count is usually seen in allergic reactions anywhere in the body. I would like to know more details:

- Does he have any allergies? Such as excessive sneezing, runny nose or skin rashes?

- Does he have wheezing or asthma?

- Does he have any abdominal pain/diarrhea?

Intestinal parasites can also cause increased Eosinophil counts. A stool test will be required if he has no apparent allergy or asthma.

As with the pain in his sole, it is likely to be unrelated and a simple soft tissue pain. Hot water compress should provide relieve.

with respect to mail, we submit that:

Intestinal parasites can also cause increased Eosinophil counts. A stool test will be required if he has no apparent allergy or asthma.

Reaction : PH 7.0
MUCUS : NIL
BLOOD : NIL
Microscopic Examination:
PUS Cells : 3-4 / HPF
R B Cells : NIL
E P Cells : 3-4/ HPF
CYST : NIL
OVA : NIL
Others : FAT GLOBUL PRESENT

Let me address your queries one by one. To go back to your old consult, eosinophils are mostly raised by a minor fraction, if the child has some allergic manifestation. In your case your son, might be having some food allergy, and that can also lead to eosinophilia. There is nothing to be worried about the eosinophils as of now.
Now coming to the stool report, everything is fine apart from the fat globules in the stool. Normally presence of fecal fat indicates that the patient has some malabsorption, mostly not being able to absorb the dietary fat. In his case, are his stools pale white in colour? Is it extremely foul smelling? Does he have a fatty diet? I will advise you to give him more green leafy vegetables, try to include more pulses and legumes in his diet. For meat lean mean like chicken is preferable till we address his malnutrition issue. Just avoid too much oily food and make sure your son is eating properly for the next few days. He can also have an intestinal infection, which can raise eosinophils, and also cause malabsorption. For that i will advise you to give him T.Mebendazole 100 mg, once before sleeping when you can, and one dose again 3 days later. ( Total 2 tablets ). This will take care of the intestinal worms. Also after 7 days repeat the following tests :
1) CBC
2) Stool Routine ( For ova, cysts and parasites )
3) LFT.
