May I know where do you have this server pain ? Inside the abdomen (abdominal pain in general) or at the anus ? Is there any passage of blood in stools ? Are you having constipation ?
Also did your Doctor do a per rectal examination ? (Examining the anus using fingers ?)
This additional information will help me providing a better opinion.

Hello Doctor,
I do have pain when i pass stools. It burns for a while and when i spray water i feel relaxed. There was once a dot of blood stain when i wiped with paper. No constipation. Also no pre rectal exam done by doctor.
Please advise for an ointment and shall i continue with the medicine Azithrorite 250 as prescribed.

Hello Praveen,
This could be hemorrhoids. I do not think Azithrorite will do any benefit here. You can use a OTC anti - hemorrhoid cream called Smuth twice daily. (Follow the instruction in the leaflet). Also you may take Syrup Smuth 5ml twice daily. These medications should be taken in consult with your Doctor as we are not allowed to prescribe online.
Eat lot of fiber rich food. DO NOT strain unnecessarly in the bathroom. See a General Surgeon for evaluation.
