If only Microdochectomy has been done, the whole breast tissue has to be removed. If no other surgery was done initially, I would suggest a Modified Radical Mastectomy (MRM) procedure to be done.
The tissue report suggested a lot of in situ (non-spreading) component and 0.3 cm invasive(spreading) component of the tumor.
A final pathology report after the MRM procedure can give a better idea based on nodal involvement.
I cannot comment on chemotherapy (treatment with medications) but the patient may have to be on 5 years old patientof Tamoxifen therapy plus or minus Radiation therapy in view of close margins.
You can discuss this with your oncologist, and about the pros and cons of the treatment modalities suggested and decide.
