She don't had any major symptoms. As a follow up checkup., we had ct brain and ask to check Mri and found this. Can you please tell me is this really to worry for and what needs to be done to overcome? Is this coz of immunosuppressive medications that she having for kidney transplant. What precaution needs to be done now?

There is no need to worry about these findings, as patient is well and there are no symptoms.
Yes, these MRI changes could be related to immunosuppressive medications or transient increase in blood pressure. This condition is called as PRES (posterior reversible leukoencephalopathy syndrome) and improves on own in most people. No specific medications are needed for this condition.
