Frankly, this does NOT sound like a PCOS problem.
Women with PCOS do have irregular and delayed cycles, and irregular shedding of the lining of the uterus.
When they do have periods, they are heavy, because the shedding off of the lining that is heavy causes heavy bleeding.
But the extent and severity of the bleeding that you describe, that is not normal even considering the clinical profile of PCOS.
You might be having a fibroid, cyst, a cervical lesion or a polyp.
IN addition to an internal exam, you need a transvaginal ultrasound scan and a Pap smear at the earliest.
Please get that done, tomorrow itself if possible.
YOu just cannot go on bleeding this way.
ANd if the birth control is not helping, you might need to be on heavy progesterone dosages.

I forgot to mention, I have been experiencing lower back pain, cramping, and joint pain. I am tired no matter how much sleep I am getting, and just have no energy to do anything. I know having PCOS can put me at a higher risk for cancer, and my father is adopted so I don't know that family history. I get a little worried because I am afraid my symptoms match some of those of cancer. And I know that is probably me just being paranoid. I'm glad you agree that it doesn't sound like a PCOS issue. I know my body and something is definitely off. Thank you so much.

PCOS does increase your chances of developing endometrial cancer, but not at 21, and even at a higher age, the risk is not as high as you think.
Very few women with PCOS go on to develop this cancer - obesity, smoking , diabetes and hypertension increase this risk tremendously.
I think you are more likely to have endometrial hyperplasia or a polyp.

You might be having this symptoms due to the blood loss and resultant anemia / thyroid issue.
Get these tested too.
