Dear sir,
Thank you for your query at DoctorSpring.com
You need to get examined to rule out hernia or varicocele. Both of the conditions shall cause pain when intraabdominal pressure is increased such as during physical exercise or straining to pass stools or sexual intercourse.
If these abnormalities are not picked on physical examination then you should undergo ultrasound to look for them.

I have gone to several doctors and they ruled out a hernia and i have done a few ultrasounds and nothing was found. i was wondering if they are missing something
Dear sir
You need Doppler ultrasound of scrotum to rule out varicocele. If that also comes negative then get serum uric acid levels done.
Please get back to me with the reports.
Take care
