Chest pain with breathlessness with normal STRESS test, ULTRASOUND.

Hi I am 25 years old patientold male with no family history of heart disease. The issues I have been having started almost 2 years old patientback when on one day I suddenly started to feel uneasy & breathless with a mild chest pain. I ran to the ER in Singapore & was checked for ECG , ultrasound ,xray blood work etc post which I was discharged with a referral letter to a cardiologist. I visited the cardiologist a couple of time he did one stress test (tredmil ) & said there is nothing wrong with my heart. however even after this the symptoms continued so I started taking Xanax 0,25 mg daily as recommended by the doctor. I continued for a while however the symptoms became worst in March this year after I came back from a trip to Europe. I started to vomit every thing I eat and would feel I would faint with a chest pain and sweating so I rushed back home (India) here a saw a doctor in Fortis hospital who did a 2d echo and stress test again. The test was ok however looking at my condition he said to go for a angiography. I was scared however I went through the process I was released the same day as the result came with 0% blockage & was referred to a physiatrist. Though I was very motivated and happy with life I thought of going to physiatrist. This doctor diagnosed me with panic attack and gave me medicines for the same. I took the medicine however they were of no help.

I am still getting this symptoms I have been to ER at least 10 times with all the same procedure repeating however no doctor is being able to diagnose it. I wonder if my angiography report is reliable. I keep on have this feeling of breathlessness, Dizziness & mind pain in chest & Axilla also by mussels are tensed all times . I even went for a MRI scan of my back and neck & it was also ok

I was earlier on hypertensive medicines as my BP was borderline high however I was asked by the doctor to stop taking them as my BP was normal on all occasions it was checked. I have  borderline high cholesterol with no diabetes & all the other blood works are normal

Really need any help on this.

Going through your history and with all the test for cardiac were found to be normal including angiography shows that your heart is normal. Sometimes muscular pains , acid peptic Disease or Panic attacks can simulate heart pain. So you need to be evaluated for the same.
Since all your reports are normal, there is no cardiac problem here.

In your case i feel it is more of panic attacks and you need counseling. Do have a positive attitude to life and do try some meditation which can make you feel better. You can talk to your psychiatrist about continuing with Xanax or switching over to some other drug.

Thanks for your responce Dr Satish. The question is how can we dignose what it really is ? I have done full body check up to rule out any thing including all live and kidney tests . Is there a specific test you recomend further ? I use to be a smoker but i quit 4 months back. I have also lost 6 kgs in past few months.
I am a very positive person & love to travel. I have travelled around the world & love my Job & family however still I went to a phycatrist thinking it might be of any help but i have recived none.

Dear Doctor, I appreciate that you are very positive person. Since you lost weight during few months, is it due to excessive exercise or due to lack of appetite and is it associated with any episodes of fever.
To rule out Acid Peptic Disease you may require Oral Endoscopy to confirm it. Since you had history of smoking there is a possibilty that you may have Acid Peptic Disease. To prevent Acid Peptic Disease avoid spicy food, eat at regular intervals and you will require proton pump inhibitors like omeprazole twice a day one hr before food.
In case of muscular pain, you will feel tenderness over the site of chest pain.
If the above are ruled out , then definitely you require counselling and if this still persists then definitely you will need a pshycatrist help.

I understand your concern.
I have reviewed your entire history as well as I have gone through all your reports.
I can safely rule out any cardiac problem here, based on your 2D ECHO, as well as the treadmill test.
Angiography is an extremely sensitive test, and since it shows 0 % blockage, I would in fact say, your heart is in a great shape..
However, we can't ignore the symptoms you mentioned.
I have had similar cases before where patients have presented with chest pain, palpitations, breathlessness and a variety of other symptoms, with absolutely normal reports. Then simiarly like you, they were referred to a psychiatrist because these are associated with panic attacks, just like in your case, and all of them seemed to recover after being put on medication for the same.
The dose you mentioned for Xanax, does not seem adequate, and I would request you to get a second psychiatric opinion, or you can also discuss the option of a few sessions of CBT which is helpful in reducing such panic attacks. My diagnosis is nothing related to cardiac or pulmonary issues here, however, these anxiety disorder needs to be addressed.
