hello doctor,
my wife is recently diagnosed with PCOS and recommended gynavit syraph, myotol F and ovaflo SR tabs for six months. as i gathered info from internet, the results found are causing worry for me.facial hair, thick hair around nipples, black rashes at neck all symptoms are in growth, which are causing discomfort for me.it is been over 3months since she started taking medicines but no encouraging results. we r married a year ago. she is 25 yrs old. she is having irregular periods since beginning i.e. from 12 years of her age. she is 57kgs weight n 150cm height.now it is over 2months since her she got her last periods. thyroid test was done and it was negative. i want to ask u for opinion on
1. conceiving pregnancy
2.removal of unwanted facial hair and hair around nipples
3. periods to b regular
3. diet pattern
4. suitable treatment
5. opinion about present medication
6.any special instructions

Thanks for writing to us with your health concern.
PCOS is a condition of baseline hormone disturbance.
THere is no permanent ' cure ' for it , but lots of treatment and measures to help you along.
First of all, the investigations are not complete.
SHe should be getting a lot of hormonal tests done, apart from the ultrasound scan.
The ultrasound scan also should be done transvaginally, not abdominally, on the second day of periods.
Hormonal tests to be done on second day of cycle are - FSH, LH, TSH, Prolactin, OGTT, DHEAS, Free Testosterone, Lipid Profile.
Now regarding your other concerns -
1. Conception and pregnancy - Women with PCOS have irregular cycles, so ovulation is erratic and pregnancy can be difficult.
First of all she should lose a bit of weight. Even 2 -3 kg weight loss will help her to regulate her cycles.
Diet should include - lots of fluids, water, fresh green leafy vegetables, fresh fruits, whole grains, curd, nuts, skimmed milk, fish etc.
Avoid - potatoes, rice, fried food, processed food, excess of sugar and salt, cold drinks etc.
Exercise daily for atleast 30 minutes, for atleast 5 days a week.
You are sure to see results,
Try yoga and meditation.
Try naturally for conceiving for atleast 6 months. Since her periods are irregular, she can use Ovulation Predictor Kits or Ovulation strips to check for ovulation.
If this fails, then consult an Infertility Specialist, and go for 3 months of follicular monitoring, which involves tracking of the egg with ultrasound scan to confirm if she is ovulating ( releasing egg ). Also, semen analysis should be done
If this too fails, then more intensive treatment is needed.
A laparoscopy should be done to check if her tubes are open, and then IUI or IVF can be considered.
There are many medicines which help a woman with PCOS to conceive, such as Clomiphene and other injections and 3 months with those should be tried with follicular monitoring to check if she is ovulating.
2. Her medications are fine.
The hair should be removed via laser therapy for a permanent solution.
3. Periods will be regular with diet , exercise and the treatment that she is already taking.
Many women are given oral contraceptive pills to regulate periods, but since she wants to conceive, that is not an option.
Cyclical progesterone can be given every month to regulate periods, please discuss this option with your GYN.

5. Present medication is ideal for her.

thankyou for your reply. it was exactly the same thing which was told by the gyno...however she didnot tell about Yoga.
i would request you to mention
1. the particular asanas which are good for pcos as we dont have a professional Yoga expert neat to my locality.
2. items which should include and not to be included in diet.
Its been over 75days since she had last period. today i tested with preganews and result is negative. in this condition shold i go for pils for periods? if so, i reques you prescribe and mention dosage also.
I am planning to get all the FSH, LH, TSH, Prolactin, OGTT, DHEAS, Free Testosterone, Lipid Profile tests after completion of initial 6months medical course.

1. There are no specific Asanas for PCOS, you must begin with Basic Yoga training imparted by a professional.
Yoga is more to relax and soothe you and reduce stress, as stress can mess up your hormones.
2. I have written in detail about the diet in my initial answer, you should contact a Nutritionist / Dietician for a specific diet chart.
3. It is best not to take pills to induce menstruation without consulting your gynec and having an examination. If you wish to take the meds, you can start with Tab. Meprate 10 mg thrice a day for 5 days, once these are stopped, she will have bleeding within a week maximum.
