Hello sir, i am suffering from itching problem inside my penis foreskin and also in the head of the penis. but this is not because of dirty as there is no smegma on it. But i can see some small places in the penis head and also in the below penis head becoming white and the itching is on the same places. The itching is not anytime, it comes 3 to 4 times a day. Please help me out. I am sending u a snap of the infected place. Kindly have a look and reply to what ointment can i use and get instant cure.

I have reviewed the images and they look like fungal infection - candidiasis.
You need to start applying Clotrimazole ointment on the affected areas for 2 weeks, and you can also take One dose of Fluconazole 100 mg.
It should resolve on applying Clotrimazole. This is not because of smegma as you correctly pointed out.
