Given the size of the adnexal mass, surgery is required.
It is too big for conservative management, I mean, with medications.
However, the uterus is normal, so right now, you can go for removal of the right sided mass.
This can be accomplished laparoscopically.

Further management will depend upon the histopathology ( biopsy ) report.

Thanks for your reply,Yesterday we consultant with a doctor,she suggested uterus removal ,is it needed ? we take an appointment for CT scan tomorrow

If the CT is clear, there is no need for a hysterectomy.
Such tumours might sometimes recur in the other ovary, in that case, some doctors prefer to remove the other ovary and the uterus as well, to avoid any potential future complications.
That is also an option to be considered, and would not be a bad one.

Yes, the CT is also suggestive of a dermoid cyst which is a benign lesion.
You can opt for removal of the lesion itself or a formal hysterectomy ( to reduce any future possibility of growth on the other ovary ).
It is a valid patient's choice, and both are fine.
