Loss of muscles in hand with normal EMG,NCV.

Resolved question:
Dr,
I am 37 year old male, 6ft tall and weights 74kg. Over last few years i have been seeing gradual loss of fat/muscle mass especially on my hands and feet. Some visible shrinking is also observed on the arms and lower leg mass. (Attached are some pictures). I can walk; perform all other functions as of now. Certain movements do cause a snapping sound at wrist and also at times on the hip, knee, shoulder, elbow joints as if muscles are wasted and tendons are snapping. I get some muscle twitching at times on feet and sometimes on arms. But not for very long, like 3-4 twitches. Sleeping on certain positions gets me cramps on hand (up to fingers) at times.

I did consult a neurologist in my area and he did EMG/NCV tests which returned normal results. Next was an MRI and that too was normal except for a tiny lesion he found on one of the D blocks which as per his diagnosis is not possibly the cause for any muscle wasting is quite a small one. All other clinical tests done (kidney, liver, serum b12, thyroid etc) are normal.
Please help me with your experience with such symptoms.
Regards
Bip

Hello,
Thanks for posting your query at DoctorSpring.com
I have gone through your question in detail and I can understand what you are going through.
The kind of symptoms that you are stating suggests that the illness that you are having could be a complex partial seizure. None of the blood tests will diagnose this illness, but an EEG can demonstrate these changes.
The treatment is not very difficult and the things can be taken care of with the use of antidepileptics such as carbamazepine or oxcarbamazepine. But first I think you should go ahead with an EEG.
The condition is treatable, so please don't stress yourself much.
Kind regards

Thanks for the reply, Sir. I don't have any past medical history. Just a Kidney Stone at the age of 8 which was resolved in a week. Also, I am not having any hypertension. Just a little tension of studies which is common I guess. Also I will go will the EEG Test you have told to perform.
Thanking You.

Hello
Your kidney stones and stressors are most likely not related to the current problem of seizures. Do get an EEG done. You may let me know the results next time.
Regards.
