Hairfall in transgender after trauma and depression

Hello,

10 years ago I started suffering from hair thinning and loss in all over my head after having a trauma and depression, but in the last 5 years it started to have a sever loss where I find my hair everywhere and when I put my fingers through it, it comes out directly from all over the head
I did a different blood tests:

ESR: 64 mm/hour
DHEA-S: 2.52 umol/L
Free testosterone: 0.55 pg/mL
Zinc: 780 ug/L
Ferritin: 110.7 ug/L
FSH: 4.40 U/L
LH: 4.36 IU/L
TSH: 1.74 mIU/L
Vitamin D: 20.94 ng/mL


Many times I shaved my hair totally because I am desperate from it and the last time was 8 weeks ago and when it started to grow again I noticed when I put my fingers through my hair that there are small hair all over my head which did not grow like the rest of my hair.

What is the diagnosis for my condition and what are the suggested treatment ?

Thanks for posting your query at DoctorSpring.com. I can understand your concern.

I went through the history you have provided, and the pictures suggest that you are suffering from androgenetic hair loss which is also known as patterned hair loss.

The best treatment for it would be application of minoxidil solution on the scalp twice a day.
This would help in regaining the lost hair and would also stop further hair thinning.

However, its a long term treatment and on stopping the treatment there would be loss of the hair that has grown.

The second line of treatment in case minoxidil does not help is finasteride which has to be taken orally and would need a prescription.

I would like to request you to send me the reference range for the blood test results you have sent, as values differ at times from laboratory to laboratory.

You have not answered my Q regarding the small hair all over my head that did not grow as the rest of the hair ?

I have to mention too that one doctor diagnosed me with diffuse hair loss.. Is that correct ?

I actually answered your question regarding the short hair over the front and top of your head which didn't grow as well as the rest of the hair, I diagnosed it as androgenetic alopecia based on that finding. In androgenetic alopecia, there is thinning and shortening of hair in these areas as compared to the rest of the hair.

From the pictures and history, it doesn't seem like diffuse hair loss right now. However, it could have been diffuse hair loss earlier when you had hair loss due to a stress factor that you had mentioned.

All results are within normal limits. DHEAS is in the lower side, however since these tests were done a year ago they don't hold much relevance right now, as the levels could have changed in a year. I would recommend that you repeat the free testosterone and dheas levels.
