DUCTAL CARCINOMA INSITU, Her2 negative. Treatment options?

Dear Sir,
About two months ago i have asked you about a lump next to the nipple which mammo cannot even see, but my obgy felt it. I did a ultrasound and found a small size lump.
I actually later got the incision biopsy and unfortunatly it turns our i have stage 0-1 cancer. It is 1, Ductal Carcinoma in Situ, intermediate grade, measure 1.6cm, with positive resection margins, 2, mutiple foci of microinvasive carcinoma, ranging in size from 0.15mm to 0.3mm. Also my ER test is positive 99% 3 +, Pr: Positive, 100% 3+, Her-2: negtive 0+, Ki-67: Low 5%
The doctor will need another surgery and this time which the size about golf ball. I do not think i have any other choice but i was wondered, after the surgery (the place is right next to nippple, so the surgeon told me he will save the nipple and do a bat wing cut), so the nipple will not drop, but will the breast change shape alot (it is about 4 o'clock to the right nipple)? I know it will be dented, but will it be a lot? or over the time it might get back a little?
also the doctor said i will have 6 weeks of radiation for the breast and later also should take hormone daily. I checked seems hormone will have some side effects, some people said it will make you aging faster and look older and dry the sex drive. so does the hormone is a must thing? or i can choose not to take?
I am so sorry to ask this, i understand to cure the cancer i have to do something, but i am also a woman only 40, so i do also care about what my body will look like afterwards. I just need a headsup so i know my option and what i have to look for.

I am still awaiting your picture, but I do need additional few details from you.
- Is there any recent possible exposure to herpes?
- Is the scratch area itchy?
- Have you tried any anti-histaminic till now?
With a previous history of ingrown hair, the scratch like sore could be due to a localised inflammation around the ingrown hair area. So once I see the picture, I will be able to guide you better.
Do not worry about the number of follow ups, I will keep this consult open till all your queries have been answered.
