Resolved question:
Doctor
I am a 31 year old healthy STI free male.
Approx 10 days ago I participated in unprotected oral sex and analingus with another male. I had these acts preformed on me, I did not reciprocate.
I have no symptoms at this time.
What are my realistic concerns regarding HIV and other STI's?
Is the mouth to anal or mouth to penile route common for transmitting STI's?
I would also like to add I always use condoms when it comes to vaginal/anal/oral intercourse . This was a one time lapse, hence the concern.

Hello,
Thank you for your query at DoctorSpring.com
I understand your concern.
The only infection that can be spread via unprotected oral intercourse is herpes, provided the partneer performing the act has open herpetic lesions on his oral cavity.
Otherwise, as such the chance of spreading STIs from oral sex, is very low.
The same thing goes for analingus, apart from herpes, nothing else can be spread. Even if he did have herpes, there is not more than a 5-10 % chance of spreading the infection. There is no chance of HIV at all.
Feel free to discuss further,
Regards.
Thank you, I did not see any lesions on this individuals mouth and i have shown no symtoms so in regards to herpes I feel confident.
Is syphilis a concern? I have heard oral syphilis is rare.
Yes, syphilis is extremely rare.
So do not worry about that.
In absence of open lesions, there is nothing to worry.
Regards.
Have you ever encountered HIV transmission that has taken place with insertive oral or analingus?
Hello,
There has been no such documented cases, and I have not come across such a case in my clinical practice either.
Regards.
My last question
I heard shphilis has been on the rise in developed countries. Is it still rare even with the increase in cases? Is oral syphilis less common than penile or vaginal?
At this point would it be safe to resume sexual relations with my girlfriend?
Yes oral syphilis is rare, and it is not rising.
You can resume sexual relations.
