Resolved question:
Last year took 11 yr old son to dr bleeding gums and a nose bleed. He has always been very thin and looking pale. Blood test came back normal. He has intermittent stomach pains. And vomited twice. Bleeding gums and back pain occasionally he has always been very thin and and looks pale blood test done last week dr said she ran in depth testing she was concerned he also had some swollen lymph nodes in neck but did have some infected teeth. Blood test came back good except anaemic. He did a stool test which was positive for blood. My question DDR wants him to get a celiac test which we do have in our family. But should we get ultrasound what else could this be . does. Two blood tests rule out blood disease. Of course very worried.
Thank You

Hello Michele,
Thank you for your query at DoctorSpring.com
I understand your concern.
Most of the symptoms that you described are falling under the clinical spectrum of Celiac Disease. If blood disease has been ruled out, and with a family history of celiac disease, it most probably is due to Celiac. The Celiac disease patients will present with anemia, they will be very pale and thin, failure to thrive in severe cases, there can be blood/ fat in the stool. There will be other concurrent vitamin deficiencies, in his case he has bleeding gums, could be due to a lack of vitamin k, since it is a fat soluble vitamin, and celiac auto antibodies, does not let fat being absorbed in our body.
But celiac has no relation with blood in urine. Did you get a urine test done? If so can you please upload the reports?
For celiac disease, we rely mostly on the history ( family history ), anemia ( serum iron levels need to checked), stool ( for occult blood and fat ), and the only confirmatory test is a biopsy from the small intestine. But in most cases if there is a family history, with anemia and other signs of nutritional deficiencies, we diagnose the patient with Celiac.
The swollen lymph nodes again is mostly due to the infected teeth. Has any antibiotics been prescribed?
Also could you attach the blood reports? And any other relevant reports that you have, so that i can go through and help you further,
Regards.
Thank You so much for your help. I don't have any of the test available but the urine test showed
trace amounts and then a week later it showed no blood. IIf the celiac test comes back negative.
his doctor will refer us to gastroenterologist . should we do colonoscopy and ultrasound to be sure of
celiac diagnosis.
Thank You !

Hello,
So at that point, he probably had an acute UTI.
You can do another urine test just to make sure there are no further RBCs prophylactically.
I will recommend doing an ultrasound also to rule out other organ enlargement such as liver/spleen.
Colonoscopy won't be required as of now.
But i will suggest you get a medical gastro opinion as soon as you can.
Hope this was helpful,
Regards.
