Resolved question:
Following the death of my wife I suffered a deep and lengthy depression. The Psychologist determined that instead of a Manic phase I just returned to normal. During that time and unknown to the Psychologist I was experiencing sexual deviant desires. Bondage, Castration, Restricted breathing etc, could this be the Manic phase he was looking for.These deviance's are strong and usually work up to conclusion excepting Castration, though that is nearing with Elastration with rubber rings. I see my Psychiatrist next month, what is he likely to want to do. Together with an excessive desire to sleep, my life is being adversely affected. Is my problem serious, or at least serious enough to take me out of the way for a while? My GP has had an extensive blood test taken and all is normal, as is my testosterone levels of 8.1 nmol/L whilst serum sex hormone binding glob is at 20 nmol/L

I have gone through your question in detail and I can understand what you are going through.
The sexual deviant behaviors can occur along with depression and it is better known as a mixed episode in that case. Mixed episodes are said to be presence of depression along with symptoms of mania like hyper sexuality in your case. The situation in which you are in is dangerous and there is always a possibility of harming yourself. Its always good for you to be in a constant observation of a mental health professionals in such a situation.
Lithium and naltrexone are good drugs for such a situation and help in reducing the extra sexual impulses. Your psychiatrist may want you to start on one of them.

Hi Dr Reddy, thank you for your response, however I am left in doubt. Clearly I am a risk to myself of Self Harm particularly in castration. You spoke about constant observation, and also of Lithium. Does that mean that a period of inpatient is likely or periodic observance and Lithium.?

You will require an inpatient stay and you need to be put on lithium while staying inpatient.
